How to request certified copy of California Vital Records from CDPH Vital Record Office?

CDPH accepts requests for certified copies through the mail. Follow these 5 steps to request your order fast and easy:-

1. You have to obtain a  ” Certified Authorized Copy of Birth Certificate” if you want to Apostille it after that.  Know more about Authorized copy VS. Informational Copy.
2. Complete Application for Certified copy of Birth – Death – Marriage Certificate- Do not forget to complete the  ” Sworn Statement Section in each Application.
3. Notarize the ” Sworn Statement ” in front of a licensed Notary Public in the State of California.
4. Prepare the appropriate fee for your request. You can pay using Check or Money Order, payable to “ CDPH Vital Records“. Please be noted that the following fee is per single document: Birth Certificate $25, Death Certificate $21, Fetal Death Certificate $18, Public Marriage Certificate $15, Dissolution of Marriage Certificate $14, Still Birth Certificate $24.
